La dosis inicial o el incremento de la dosis de
insulina la obtendremos a partir de dos variables que sabemos que aumentan la resistencia insulínica, como son el
peso del paciente y la dosis de corticoides administrada20,21,32 (tabla 4).
Posteriormente, la dosis de insulina se ajustará
según las glucemias capilares, con incrementos/reducciones
en torno al 20%. Los cambios en la dosificación de los glucocorticoides requieren ajustes paralelos

En los pacientes ambulatorios es primordial instruir al paciente o familiares en el ajuste de la dosis de insulina según la glucemia y las modificaciones de la dosis de glucocorticoides.

por la pauta hipoglucemiante previa del paciente
y por el tipo y pauta de glucocorticoide
instaurada (fig. 1, tabla 5).
